Our Staff:
Constant Care provides a wide spectrum of services that are provided by a variety of health care professionals in your home. Coordinated in support of your doctor's orders, our staff developes a written plan for your care which includes the services you will receive to improve and maintain your physical, mental, and social well-being. We encourage family and patient involvement along every step of the process. Listed is a brief description of our team of caregivers:
RN:
Care is coordintated with the patient's physician and provided by RN's who are dedicated to meeting the needs of our patients overall improvement. Their scheduled visits to your home would help maintain Vital Signs and Glucose Monitoring, Disease Management, Catheter Care, Medication Teaching, Wound Care and other skilled nursing needs.
HHA:
Work under the direct supervison of a Registered Nurse to assist patients with ADLs such as getting in and out of bed, walking, bathing, toileting, and dressing. All HHA's have received special training and are qualified to provide more complex services under the supervison of a nursing professional.
PCA:
Perform light household duties such as laundry, meal preparation, general housekeeping, and shopping. Their services are directed at maintaining patient households rather than providing hands-on assistance with personal care.
Live in Companions:
Provide companionship and comfort to individuals who for medical and /or safety reasons, may not be left alone at home.
